CANDIDATE PETITION FORMS

I need 600 registered voters in Flagler County to complete and return a candidate petition in order to qualify for re-election. You must be a registered voter in Flagler County in order to complete one of these.

This petition is only to qualify for re-election using the candidate petition option. It is not an endorsement of me or a commitment to vote for me.

Below please find a link to go to a PDF of my Candidate Petition Form. Please click on the image of the petition to view and print it. Once printed, please be sure to fill in:

• Your name

• Date of birth OR your voter ID number

• Street address (no PO Boxes)

• City

• County

• State

• ZIP code.

Then sign in the lower left and date it in the lower right box (this is commonly overlooked). Please be sure the form is complete and accurate, or it will not be accepted by the Supervisor of Elections office.

Once complete, you can mail it back to me, drop it off at my residence or contact me and I will pick it up! Please get all your family, friends and neighbors to fill one out!
